You must obtain a Return Authorization Number (RA#) and fill out the Credit Return form from the
Bio-Life, LLC. customer service department prior to any product returns.  Please contact customer service

Product returned must be unopened and undamaged product in its original packaging only.  Surgical-Prosthetic Kits
may only be exchanged as unopened, complete kits. All returned product must be in good condition with the manufacturer’s
seals intact.
 

Only individually purchased components may be returned, if components are part of the “Bio-Life Kit” they  are not
returnable or refundable due to quality manufacturing standards. 

Customers may return product up to 30 days after original purchase for in-house credit (excludes shipping). 
All returns are subject to inspection and may be returned to you if damaged or opened.  Returned product
may be subject to a 20% restocking fee. Credit will be issued after the product is inspected by Bio-Life manufacturing
inspectors.  Credit will be applied to future orders.  No cash refunds will be issued on return items.   

Returns after 30 Days, are only eligible to receive 50% Credit applied to future orders only.  No returns will be accepted
beyond 3 months of original invoice date. 

To exchange implant kits for another size, product must be returned and approved for credit PRIOR to shipment of new
merchandise.  All new or replacement orders will be billed upon shipping.  Return merchandise will create a credit
balance available for use against future purchases.
